Metronidazole (CAS No. 443-48-1) is a classic nitroimidazole antibiotic and antiparasitic drug. As an Essential Medicines list drug recognized by the World Health Organization, it has been used clinically for decades and plays a significant role in the field of anti-infective therapy due to its unique mechanism of “selective toxicity.”
Pharmacology and Mechanism of Action
Metronidazole is a prodrug, and its core mechanism of action lies in its “selective targeting” of specific microorganisms.
Selective Activation: The drug molecule itself is inactive, but it can be reduced by specific enzyme systems (such as ferrooxoreductases) within anaerobic bacteria and protozoan cells.
DNA Damage: Active intermediates generated during the reduction process (such as free radicals and nitroso derivatives) attack the DNA of microorganisms, causing breaks in the helical structure and damage to the double helix. This inhibits nucleic acid synthesis and ultimately leads to the death of bacteria or protozoa.
Narrow Spectrum of Activity: Since aerobic bacteria lack the ability to reduce this drug, metronidazole is highly selective against anaerobic bacteria and is essentially ineffective against aerobic or facultative anaerobic bacteria.
Clinical Applications
Due to its unique mechanism of action, metronidazole has a wide range of applications, covering multiple fields such as infectious diseases and parasitic diseases.
Treatment of Anaerobic Infections: This is one of its primary uses. It is used to treat abdominal infections, female reproductive tract infections, lower respiratory tract infections, skin and soft tissue infections, sepsis, endocarditis, and central nervous system infections caused by anaerobic bacteria such as Bacteroides fragilis, Clostridium spp., and Peptostreptococcus spp.
Treatment of parasitic infections: It serves as a first-line drug for treating trichomoniasis, amebiasis, and giardiasis.
Other applications: Clinically, it is also used in combination therapy for the eradication of Helicobacter pylori infections and as a radiosensitizer to enhance the sensitivity of hypoxic tumor cells to radiation therapy.
Metronidazole is widely used, primarily due to its potent bactericidal activity against anaerobic bacteria and specific protozoa. It is employed across three main areas and administered via various routes—including oral, intravenous, topical, or suppository forms—depending on the type and site of infection.
1. Treatment of Anaerobic Infections
This is one of metronidazole's most important clinical applications. It is effective against most obligate anaerobes (such as *Bacteroides fragilis* and *Clostridium* species) but is ineffective against aerobic or facultative anaerobic bacteria. It is commonly used to treat or prevent infections in the following areas:
Abdominal cavity and gastrointestinal tract: Peritonitis, intra-abdominal abscesses, and liver abscesses.
Female reproductive system: Pelvic inflammatory disease (PID), endometritis, and bacterial vaginosis.
Lower respiratory tract: Necrotizing pneumonia and lung abscesses.
Central nervous system: Meningitis and brain abscesses.
Other: Sepsis, endocarditis, bone and joint infections, skin and soft tissue infections, and periodontal or oral infections. It is also used for prophylaxis prior to intestinal surgery.
2. Treatment of Protozoal Infections
As an antiprotozoal agent, metronidazole is a first-line treatment for various protozoal infections.
Trichomoniasis: The primary drug for treating trichomonal vaginitis.
Amebiasis: Used to treat both intestinal and extra-intestinal amebiasis (e.g., amoebic liver abscess).
Giardiasis: An effective treatment option.
3. Other Applications
*Helicobacter pylori* eradication: Used as part of combination therapy regimens for *H. pylori*-associated gastritis or gastric ulcers.
Skin conditions: Topical formulations are used to treat rosacea.
Specific infections: Can serve as an alternative treatment for mild-to-moderate *Clostridioides difficile* colitis and is used for infections such as Guinea worm disease (dracunculiasis).
